PLEASE NOTE THAT WE ARE NOT ABLE TO ACCEPT MEDICAID AS A SECONDARY INSURANCE UNLESS YOU ARE ON MEDICARE OR HAVE A MEDICARE ADVANTAGE (REPLACEMENT) PROGRAM.
WE DO NOT ACCEPT HEALTH PLAN OF NEVADA OR HUMANA POLICIES AT THIS TIME.

For new patients to our office:

When you arrive for your first appointment, the receptionist will hand you the paperwork that we require.
To save time, download the applicable paperwork using the links below. Please read the Notice of Privacy Practices. 

New Patients:

Please arrive 15 minutes prior to your appointment time.

What you need to bring to your first appointment:

  • Proof of Medical Insurance.

  • List of prescribed and over the counter medication you use.

  • Completed paperwork.

Please wear comfortable clothing as you may receive some treatment following your evaluation.​
